The CANS-NY Training and Technical Assistance Institute is a statewide collaborative supported by the New York State Department of Health and the Center for Innovation in Population Health at the University of Kentucky. The collaborative is facilitated by The John Praed Foundation. Regional leads in the collaborative include Astor Services for Children & Families, The Collaborative for Children & FamiliesHillside Family of Agencies and Northern Rivers.

Members of the collaborative are committed to providing coaching, training, and support for implementation of TCOM (Transformational Collaborative Outcomes Management) and the CANS-NY in Health Home Care Management for children across New York State. The intellectual property rights for the CANS are held by the Praed Foundation to ensure that it always remains free to use. The collaborative partners of the CANS-NY Training and Technical Institute are advised by an Impact Board consisting of individuals with lived experience of care, leaders who support transformation across our State, and regulatory partners from the New York State Office of Children & Family Services (OCFS), the New York State Office of Mental Health (OMH), the New York State Office of Alcoholism & Substance Abuse Services (OASAS), and the New York City Administration for Children’s Services (ACS).

The Collaborative

The Collaborative consists of lead provider agencies with substantial experience in use of the CANS-NY. Together, the provider agency leads will use their collective experience, lessons learned, and expertise to identify technical assistance needs in the state and provide support for these needs. The partner agencies, Hillside Family of Agencies, Northern Rivers Family of Services, Astor Services for Children & Families, and will have statewide and centralized responsibilities for the development of Institute products for training and coaching. The map below identifies the geographic “coverage” area for training and coaching:

Each agency was selected because they have been a leader in their region for effective use of the CANS. Follow the links below to find out more information on the history of CANS-NY implementation in these select agencies and their ongoing commitment to improving outcomes for children and families in New York.

Impact Board

The Impact Board is charged with helping to guide the CANS-NY Institute’s progress toward its mission. Impact Board members are asked to bring their knowledge of how the CANS-NY is being experienced by youth and families, how the CANS-NY is being used to guide care, and what training and coaching supports are needed to increase effective use of the CANS-NY.  CANS-NY Institute leadership provides monthly email updates to the Impact Board about Institute progress and findings, and will coordinate one in-person and one web-hosted meeting of the Impact Board each year to elicit input on emerging learning needs, Institute effectiveness, and consumer perspective on the CANS-NY.
